If you require an appointment to take your COVID-19 Test, please follow the 2 steps below
STEP 1 – Epidemiological form from the Ministry of Health of Costa Rica
- Click here to access the form
- Fill out the form properly for each person taking the test. Access a guide on how to fill out the form here
- Save the code (this code will be required at the time of your COVID-19 test)
STEP 2 – Schedule your appointment
- Complete the appointment request
- Assist to the designated location at the agreed time and date with your original passports
**Please consider that all payments should be made with credit card to the Doctor.

Si requiere una cita para tomar una Prueba de COVID-19 Test, ppor favor siga los siguientes pasos:
PASO 1 – Formulario epidemiológico del Ministerio de Salud de Costa Rica
- Haga click acá para accesar el formulario
- Llene debidamente el formulario, el formulario es individual. Accese una guía de como llenar el formulario acá
- Guarde el código (este código será necesario en el momento de la prueba COVID-19)
PASO 2 – Programe su cita
- Complete la solicitud de la cita
- Preséntese el día de su cita a la hora y lugar definido con sus pasaportes originales.
** Todos los pagos deben ser realizados con tarjeta de crédito directamente con el Doctor.
